CONTACT REGISTRATION FORM

This allows you to create a new contact.

To return to the previous page, click Cancel.

ORGANIZATION NAME System displays the name of the contact person's organization.

REGISTRATION DATE

System displays the contact person's registration date.
ROLE Please select the role of the contact person using the drop-down menu.
STATUS DATE

System displays the date that the role of the contact person was last updated.

USER ID This will be automatically generated by the system.
SALUTATION Please select the contact person's salutation using the drop-down menu.
FIRST NAME Please enter the contact person's first name.

MIDDLE NAME

You may enter the contact person's middle name.

LAST NAME

Please enter the contact person's last name.
DESIGNATION Please enter the contact person's designation.
TELEPHONE NUMBER Please enter the contact person's telephone number in this format: country code, area code, telephone number and extension number.
MOBILE PHONE NUMBER You may enter the contact person's mobile phone number.
FAX NUMBER You may enter the contact person's fax number.
EMAIL ADDRESS Please enter the contact person's email address.
REGION Please select the region from which the contact person is from using the drop-down menu.
PROVINCE Please select the province from which the contact person is from using the drop-down menu.
CITY/MUNICIPALITY Please select the city/municipality from which the contact person is from using the drop-down menu.
STREET ADDRESS Please enter the contact person's street address.
ZIP CODE You may enter the contact person's zip code.
